Please list the child’s:

1) Full Name

2) Date of Birth

3) Exact date of Baptism (if known)/ or year of Baptism

Parents may collect the Certificates on Friday evenings from the Sacristy of St. John the Baptist Church between 6 - 6.30pm. 

Requests for Certificates to collect on the Friday evenings should be submitted by 5pm on the Wednesday of the same week. Requests left on the office telephone or Parochial Houses WON’T be dealt with. Emails will be sent an acknowledgment email on receipt of the request.

This protocol is necessary to ensure no requests are missed.
